Sweet Tooth is the main antagonist of Twisted Metal 4, being the current head of the Twisted Metal tournament, having overthrown Calypso. He is more of a ringmaster than a clown in this installment, but oddly remains silent for the entirety of the game, having one of his clown henchmen speak for him.

He is the final boss, and is considered one of the most difficult bosses in the series. In the same vein, Sweet Tooth's special attack in TM4 is considered the most overpowered and broken of any Special Weapon in the entire series.

Story (Intro)

When he was a child, the Twisted Metal competition - with Calypso at its head - came to his town. The contest was unlike anything Sweet Tooth ever laid eyes on. It gave him dreams of death and destruction, and so he followed the traveling tournament until he was able to enter as a contestant. Sweet Tooth proved quite adept in combat and killed many opponents, but eventually grew dissatisfied. He felt that he was just a pawn, killing while Calypso grew stronger with each soul he collected. With this realization, he won the next tournament and made a wish to be the star of Twisted Metal. Now, after overthrowing Calypso, Sweet Tooth rules the deranged demolition derby from his carnival-like lair, assisted by a horde of equally twisted clown minions. His newly-upgraded ice cream truck is the final challenge for anyone seeking to make a wish, but his sense of humor is just as bad as Calypso's when it comes to giving winners what they wanted.

Ending

Because he is the final boss, Sweet Tooth doesn't have a unique ending, instead sharing the generic ending with the other bosses and custom cars.

Trivia

• He is only referred to as "Sweet Tooth" throughout the entirity of Twisted Metal 4, completely dropping the Needles Kane moniker.
• Despite being the lead character in the game, Needles never speaks, instead using Melvin as an inbetween to talk to the player character.
• There could have been a possibility of Needles having an info screen, as his TMIII info screen can be found in the game's files under the name "STOOTH.TIM[0]."
• Magazine ads for Twisted Metal 4 depict a live photo of his truck being cleaned outside a carwash by Sweet Tooth's clown assistants. Sweet Tooth himself can be seen sitting in a chair nearby reading a magazine.
